Chapter 69: Vaccines and Immunizations

Which of the following can safely be given to a 6-month-old child who had an allergic reaction to the pertussis vaccine?

a. DTaP

b. Tdap

c. Td

d. DT

Answer d is correct. DT is indicated for the prevention of diphtheria and tetanus in children less than 7 years of age. It does not contain the pertussis vaccine. It should be used in children who have previously had an allergic reaction to the pertussis vaccine.

Answer a is incorrect. DTaP is indicated for the prevention of diphtheria, tetanus, and pertussis in children less than 7 years. However, it contains the pertussis vaccine and should not be used in a patient who has had an allergic reaction to the pertussis vaccine.

Answer b is incorrect. Tdap is FDA approved for use as a single booster dose in adolescents and adults between the ages of 11 and 64 years. ACIP recommends it for patients as young as 7 years and adults over the age of 65 years as a single booster dose due to the current pertussis outbreak. It also contains the pertussis vaccine. It should not be used in a 6-month-old who is allergic to the pertussis vaccine.

Answer c is incorrect. Even though Td does not contain the pertussis vaccine, it is indicated for use as a booster dose in adolescents and adults over the age of 7 years. It would not be appropriate to use this vaccine in a 6-month-old child.
