The purpose of this activity is to allow students to practice their verbal communication via telephone with a patient’s family member to obtain an accurate medication list. Students will then communicate this information with the provider in the form of a pharmacy note.

Learning Objectives
  1. Collect a patient’s home medication list from multiple sources from an electronic health record (EHR).

  2. Identify medication discrepancies between multiple patient medication lists.

  3. Resolve medication discrepancies through telephone communication with a patient’s family member.

  4. Formulate an accurate home medication list based on information obtained from the patient’s family member.

  5. Utilize patient-centered communication skills to gather information from the patient’s family member.

  6. Document the correct home medication list in the EHR.

  7. Communicate changes made to the patient’s home medication list and concerns with patient adherence in a care plan note within the EHR.

  8. Perform self-assessment and peer-assessment skills.

Assignment Instructions

Each student individually reviews the Patient Case prior to completing a phone call with the patient’s family member. After students have individually completed the phone call and gathered information from the patient’s family member, the students then work together in a team to compare information and complete the medication documentation using the Medication Reconciliation Note Template as a guide.
