Your role is to follow the patient-centered care process to determine if the patient is appropriate for self-care treatment and if so, to make a recommendation.

  1. COLLECT

    1. Utilize SCHOLARMAC.

  2. ASSESS

    1. Differentiate signs and symptoms and identify patient’s primary problem.

    2. Determine if the patient is an appropriate candidate for self-care.

  3. PLAN

    1. Create a care plan including both pharmacologic and nonpharmacologic self-care therapies.

    2. Describe recommendation and rationale.

  4. IMPLEMENT

    1. Provide patient education or counseling—including information on medication selection, dose, administration, monitoring, expected outcomes, common adverse effects, and clinical pearls.

  5. FOLLOW UP
