INTRODUCTION

About the Authors: Matthew Grissinger, RPh, FISMP, FASCP is the Director of Error Reporting Programs at the Institute for Safe Medication Practices (ISMP). He works with health care practitioners and institutions to provide education about medication errors and their prevention and review medication errors that have been voluntarily submitted by practitioners to a national ISMP Medication Errors Reporting Program (ISMP MERP). He has published numerous articles in the pharmacy literature, including regular columns in Pharmacy and Therapeutics (P&T) and the Pennsylvania Patient Safety Authority’s (PSA) Patient Safety Advisory and is a journal reviewer for several publications including the Joint Commission Journal on Quality and Patient Safety, Pharmacoepidemiology and Drug Safety, and Annals of Internal Medicine. He is a chapter contributor to a textbook published by McGraw-Hill entitled Pharmacy Management: Essentials for All Practice Settings, Remington and Medication Errors.

Mr. Grissinger serves as the Chair for the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP) and serves on the National Quality Form (NQF) Common Formats Expert Panel, the Editorial Board for P&T, the Faculty Advisory Board for the Pharmacy Learning Network (PLN), and the Publications Advisory Board for Davis’s Drug Guide for Nurses. He also served on the United States Pharmacopeia’s (USP) Safe Medication Use Expert Committee from 2005 to 2010, the Food and Drug Administration (FDA) Proprietary Name Review Concept Paper workshop panel in 2008, FDA Naming, Labeling, and Packaging Practices to Minimize Medication Errors workshop panel in 2010, and the Joint Commission Home Care Compounding Pharmacy Technical Advisory Panel in 2013.

Michael R. Cohen, RPh, MS, ScD (hon.), DPS (hon.) is founder and president of The Institute for Safe Medication Practices. Dr. Cohen’s passion for medication safety began in 1974 when he was involved in a serious adverse event with insulin in the hospital where he was employed. He immediately saw the value in sharing his story with other hospital pharmacists to prevent the same error from occurring. Dr. Cohen founded ISMP in 1994 and launched the first of five ISMP Medication Safety Alert! publications in 1997. The newsletters along with Safety Alerts now reach over a million health professionals in the United States as well as regulatory authorities and others in over 30 foreign countries.

Dr. Cohen is active in many patient safety initiatives including coeditor of the ISMP consumer website, Chairperson of the International Medication Safety Network, former member of the US FDA Drug Safety and Risk Management Advisory Committee (DSaRM) and the Nonprescription Drugs Advisory Committee (NDAC) and is currently a consultant to FDA.
