zero to four point scale: Reflexes are graded on a scale with zero = no response, 1 = somewhat diminished to low normal response, 2 = average or normal response, 3 = brisker than average but not necessarily indicative of disease, 4 = very brisk, hyperactive, with rhythmic oscillations between flexion and extension (clonus).
