A CMR is an interactive face-to-face or telehealth consultation to gather and review a patient's complete medication use (including prescription medications, over-the-counter (OTC) medications, herbal therapies, and dietary supplements) and is conducted by a pharmacist or qualified provider. The CMR is intended to aid in assessing medication therapy, identifying drug-related problems, and optimizing patient outcomes. The PML and MAP are created following the CMR and provided to the patient. The PML contains, at a minimum, the medication name, prescriber, indication, and dosing regimen. The MAP provides written material that may support the CMR discussion, provide actions the patient should take to help self-manage medications as well as disease state, and help clarify therapeutic goals. MTM is outside the usual prescription dispensing and OBRA counseling processes.
